Before you request
Your questions, answered.
Can I book immediately or receive service tomorrow?
We are accepting requests now. A submitted form is an inquiry, not an appointment. We confirm supply delivery, laboratory arrangements, availability, and collection requirements first. Please do not rely on service for a deadline until you receive written confirmation.
Is the mobile fee charged for every person?
No. One mobile visit fee applies when donors are collected together at the same location. Example: five routine DOT urine tests within 10 road miles during standard hours are estimated at 5 × $75 + $35 = $410, before any disclosed extras.
Do you provide instant DOT results?
No. DOT urine specimens are sent for laboratory testing and MRO review. Turnaround varies with shipping, laboratory processing, and any required review. We do not guarantee a result date.
I already paid for a test through my employer. What do I select?
Request “collection only” and tell us that you have an existing laboratory/program order. We verify compatibility, supplies, shipping, and billing before scheduling. Do not place order numbers or personal medical details in the general form.
Can DNA collection be used for court or immigration?
Tell us the purpose in general terms. The receiving laboratory and authority determine the required process and acceptability. We must confirm the laboratory-directed arrangements before agreeing to legal or immigration-related collection. Collection alone does not guarantee legal acceptance.
How do payment, cancellations, and additional fees work?
The written quote provides payment instructions, cancellation/rescheduling terms, and any proposed dispatch or additional-service charges before you approve. This website does not charge your card or take a deposit. Do not send payment until you receive confirmed instructions.
Can I send identification or medical records through the form?
No. Please do not submit Social Security numbers, dates of birth, identification images, medical details, test results, or confidential case documents. We provide instructions for appropriate information exchange when needed.
Can laboratories, TPAs, and employers request an ongoing contract?
Yes. Choose “Partnership / Contract Inquiry” on the request form. Describe your coverage needs and expected volume without donor information. Contract scope and rates are agreed separately.
